About the Role

This role requires frequent travel (up to 90 miles one way) and in-home visitations with clients and their families. The Lead Care Manager serves as a trusted guide and advocate, providing central support for youth and families seeking care coordination and community connections. This involves close collaboration with an extended ECM team, partnering with managed care plans, and building strong relationships to ensure youth and families receive necessary services.

Responsibilities

Engage in outreach to enroll new clients in ECM and build rapport with members.
Complete comprehensive assessments and develop care plans for enrolled members.
Organize care activities and maintain contact with outside providers for coordination, including county substance use disorder and specialty mental health providers.
Manage referrals, coordination, and follow-up for needed services and supports.
Provide education and identify support needs for youth and their family or caregivers.
Deliver services in person and/or via telehealth to meet care plan needs.
Utilize crisis communication and de-escalation techniques per Seneca's crisis response training, potentially including physical management of clients' behavior.
Engage in community outreach to identify supports and resources, building relationships with community providers.

Requirements

Minimum age of 21 years.
Ability to drive within the community for client meetings and job duties at multiple locations.
TB test clearance, fingerprinting clearance, and other state/federal licensing or certification requirements.
Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy.
Own vehicle and current automobile insurance.
Prior case management and community-based experience (preferred).
Lived or family experience navigating medical and mental health, probation, child welfare, and/or education systems and resources (preferred).

Experience Level

1 year of applicable work experience with an Associate degree or equivalent.
2 years of applicable work experience with a High school diploma/GED.
Applicable experience includes paid or unpaid work with youth, families, or in the social services field.

Salary/Compensations

Starting at $26.22 - $28.72 per hour.
Approximately $54,537.60 - $59,737.60 annually.
Actual salary dependent on creditable experience above minimum qualifications.
