About the Role

Ensure the safety and well-being of all participants enrolled in our community-based program. Act as a teacher, role model, and support, providing a caring, enabling, empowering, and supportive setting that promotes increased independence. The role operates under the direct supervision of the Operations Director, Assistant Director Day Habilitation Program Coordinator, and program without walls administration team.

Responsibilities

Assist, supervise, support, and instruct individuals to promote independence and community integration.
Teach life safety training skills.
Teach communication skills.
Teach cooking, budgeting, and shopping skills.
Teach money management skills.
Teach socialization skills.
Facilitate new and existing recreational activities.
Facilitate individual and group social, health-related, and recreation activities.
Assist in the development and implementation of habilitation plans and goal development.
Complete daily documentation accurately and timely, reflecting progress on goals and other required documentation, including Range of Scan, supervision, and trip forms.
Provide safe transportation to and from the day program and for community outings.
Collaborate effectively with supervisors and co-workers.
Follow management instructions and maintain accountability for whereabouts throughout the day.
Maintain a safe, clean, and stress-free environment.
Maintain and complete all required trainings.
Perform other duties as assigned to ensure day program operation.
