About the Role

The London HART Hub is a low barrier recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs. It provides high-quality, evidence-informed, trauma-informed, culturally appropriate, and compassionate care. The Hub connects individuals with a continuum of care, including transitional supportive housing, basic needs, healthcare, peer supports, employment, income supports, and mental health and addiction services. Counsellors develop therapeutic relationships, provide addiction treatment planning, increase client motivation, and offer addiction counselling to improve clients' bio-psycho-social-spiritual health.

Responsibilities

Participate in Intake Services.
Administer and interpret evidence-informed screening tools to determine program eligibility and level of care.
Recognize and respond effectively to individuals, groups, or communities in crisis.
Provide crisis intervention and de-escalation.
Adapt services to clients’ social and cultural needs, offering accessible communication options.
Assess client transportation needs and provide travel assistance.
Utilize standardized provincial assessment tools for suitability and level of care.
Adhere to quality assurance measures for provincial assessment tools.
Provide clinical interpretation of assessment tools and use motivational interviewing for client-centred treatment plans.
Conduct ongoing re-assessments to evaluate treatment progression and adjust plans.
Develop and maintain therapeutic relationships with clients, adhering to high ethical standards.
Increase client motivation using a Stages of Change paradigm and motivational interviewing.
Apply evidence-informed counselling styles and techniques to improve clients’ bio-psycho-social-spiritual health.
Utilize best practice, promising practices, and evidence-informed models in individual or group settings.
Develop formalized treatment plans including case management, goal setting, counselling, referrals, advocacy, and education.
Evaluate and update treatment plans as appropriate.
Match clients with appropriate services based on screening, assessment, referral, and treatment planning.
Manage a clinical caseload and meet direct service requirements.
Facilitate community treatment groups as needed.
Facilitate successful service navigation for clients through empowerment or advocacy.
Participate in and lead client-centred case conferences.
Coordinate support for individuals involved in the social justice system (pre-release, probation, bail).
Provide support and education on addiction treatment, including types of addiction, treatment continuum, and continuum of use.
Provide rapid connection to housing stability resources.
Offer addiction and mental health services and support information in person, by phone, or other means.
Educate clients and concerned others on coping techniques and strategies.
Deliver outreach services for prevention, health promotion, early intervention, treatment, and aftercare.
Engage in community development, awareness, education, and outreach, representing the agency professionally.
Participate in individual and group clinical supervision to improve client care and staff competence.
Participate in administrative supervision to ensure compliance with agency and regulatory bodies.
Provide peer support.

Requirements

Police Vulnerable Sector Check completed within the last six months.
College Diploma or University Degree in a related field (e.g., BSW, post graduate diploma in addictions or equivalent).
Registration with a professional college (e.g., OCSWSSW, CNO, CRPO, CPSO, COTO, CPO).
Valid Driver’s License (minimum G2 class).
Personal vehicle for client transportation.
Two million-dollar ($2,000,000.00) liability insurance for personal vehicle.
Demonstrated ability to attend work regularly.
ASIST Training (preferred).
Non-Violent Crisis Intervention Training (preferred).
Clinical work experience in substance use treatment, including intake services (preferred).
Experience working with individuals with no fixed address (preferred).
Experience in crisis intervention/management and crisis resolution (preferred).
Experience and detailed knowledge of mild, moderate, and serious mental illnesses (preferred).
Knowledge of Psycho-social Rehabilitation (preferred).
Extensive knowledge of London's support/treatment services network and regional services (preferred).
Knowledge of the legal system (preferred).
Knowledge and understanding of philosophies, practices, models, and frameworks of Ontario’s London Hub plan (preferred).
Bilingual in both official languages (preferred).

Salary/Compensations

Starting rate: $27.14 per hour (Level 1 – Direct Service)
Starting rate (regulated): $30.01 per hour (Level 1 – Direct Service)
Grid progression up to Level 8: $34.41 per hour (non-regulated)
Grid progression up to Level 8: $37.26 per hour (regulated)
