About the Role
IOA develops revolutionary healthcare models to help aging adults and adults with disabilities maintain quality of life and age in place. The Home Care Worker provides direct services to frail elders in their homes under the Community Services Department.
Responsibilities
- Provide personal care and assistance to frail elders in their homes to maintain and increase their ability to live safely and comfortably.
- Perform homemaker services and assist with personal care including colostomy care, bathing, peri-care, mouth, skin, and hair care.
- Assist participants with eating, toileting (including bedpan use), and ambulation.
- Assist with participant laundry, cue glucose tests, BP checks, and collection of urine/sputum.
- Escort and assist clients to, from, and at medical appointments.
- Provide behavioral/emotional supervision and support, including reality orientation and reminders for nursing/medical care and diet restrictions.
- Assist in centers or clinics, including meal preparation for special diets and meal setup.
- Assist with special program/participant needs under professional staff supervision (e.g., recreation, rehab/maintenance exercise programs, internal committees).
- Ensure patient care areas are clean and safe, whether on-site or in homes.
- Report participant status and progress to homecare administrative staff.
- Fulfill administrative requirements including reporting, statistical record-keeping, and attending staff conferences and meetings.
- Participate in training and orientation of other staff and volunteers.
- Promote good public relations with participants, their families, friends, visitors, and the community.
- Perform other reasonably related assigned duties, including weekend work.
Requirements
- At least one year of experience working with older adults (paid or unpaid).
- Training and current certification as a Certified Nursing Assistant.
- Ability to work a flexible schedule, including weekends, for a 7-day service program.
- Ability to provide services in all districts of San Francisco.
- Ability to lift, transfer, push/pull, maneuver, and reposition 35 lbs.
- Ability to reach, bend, and walk.
- Ability to perform fine motor tasks with hands.
Skills

- Language capability in Spanish, Russian, Vietnamese, Korean, or Chinese.
