About the Role
This site-based role provides on-site support to residents in a housing program, focusing on engagement, crisis intervention, housing stabilization, and service coordination for individuals experiencing homelessness with complex needs. The goal is to promote long-term housing stability through individualized support, crisis intervention, harm reduction, life skills training, and connections to health and social services.
Responsibilities
- Complete client intake and initial assessments, including documentation and explanation of services.
- Deliver on-site, housing-focused case management using a harm reduction approach.
- Conduct regular on-site visits to support housing stability and prevent homelessness.
- Provide life skills support (e.g., budgeting, tenant rights, housekeeping, pet care).
- Offer supportive counseling, crisis intervention, and system navigation.
- Refer and connect clients to services including healthcare, addictions support, employment programs, and community resources.
- Advocate on behalf of clients with landlords, OW, ODSP, and other service providers.
- Build and maintain positive relationships with landlords to support housing retention.
- Maintain accurate, up-to-date records including detailed case notes and care plans.
- Prioritize safety during client interactions and service delivery planning.
- Practice trauma-informed, client-centered, and culturally respectful care.
- Participate in case reviews, team meetings, and agency initiatives.
- Contribute to a positive team culture and uphold the values of CONC.
Requirements
- Minimum 2 years of direct support experience with vulnerable populations.
- Training in de-escalation and non-violent crisis intervention (e.g., CPI).
- Knowledge of harm reduction, trauma-informed care, case management, and system navigation.
- Understanding of issues related to homelessness, mental health, addiction, and housing systems.
- Familiarity with community-based resources in Toronto.
- Strong communication, time management, and organizational skills.
- Skilled in problem-solving, conflict resolution, and team collaboration.
- Experience working with Indigenous communities is an asset.
- Experience in supportive housing or community mental health is an asset.
- Proficiency in MS Office and electronic documentation.
- Presentation of a current criminal record check.
