About the Role
This is a temporary full-time pharmacist position reporting to the Clinical Manager, supporting ambulatory clinics including Osteoporosis, Rheumatology, Gastroenterology, and Menopause. The role involves clinical and drug distribution functions within an academic, ambulatory care hospital setting.
Responsibilities
- Collaborate with patients and teams to develop and monitor comprehensive pharmaceutical care plans
- Perform medication reconciliation and provide direct patient care through prescription counseling
- Determine therapeutic options to resolve or prevent drug-related problems
- Perform pharmacokinetic consultations and therapeutic interventions
- Contribute to chronic disease management models and assist with drug coverage applications
- Act as a consultant to physicians and nurse practitioners regarding medication management
- Educate team members on drug-related guidelines and evidence-based research
- Participate in teaching pharmacy residents, medical residents, and students
- Conduct health promotion through patient counseling and education
- Interpret physician orders and perform final product checks
- Monitor patient profiles for drug indication, effectiveness, safety, and cost compliance
- Participate in on-call responsibilities
- Utilize Epic and Willow Inventory systems
- Promote evidence-based practice and participate in quality and safety research
- Participate in hospital committees, policy development, and accreditation work
Requirements
- Current registration with the Ontario College of Pharmacists with Part A licensure
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree
- Must reside and be legally eligible to work in Ontario
- Completion of Pharmacy Practice Residency Program is an asset
- One year of work experience in ambulatory care is preferred
- Completion of Epic and/or Willow Inventory training is an asset
- Current membership in a professional association is an asset
- Demonstrated satisfactory work performance and attendance record
- Knowledge of electronic medical record documentation
