About the Role
As the Case Manager, you will play a pivotal role in our organization, assisting families in providing them with the necessary care and resources needed. Join us and be a part of an organization that values your contribution and cares deeply for the community we serve.
Responsibilities
- Maintain and update digital and physical records of all families as required by governing agencies.
- Collaborate with clients to develop service plans based on an assessment of each family’s needs.
- Assist families with connecting to community resources, support groups, and cultural services to provide social support.
- Assist families with referrals to outside agencies for legal services, education, parenting, medical, and mental health services.
- Maintain and update case files bi-weekly to document progress.
- Adhere to all city and state guidelines for maintaining documents.
- Conducts unit inspections.
Requirements
- Fluency in languages other than English, particularly Spanish strongly preferred.
- Knowledge of Outlook, Word, and Excel
