About the Role
By The Bay Health is seeking a Licensed Vocational Nurse (LVN) to provide skilled patient care and home health services under the supervision of a Primary Nurse. This part-time position emphasizes patient involvement in medical decisions and aims to maximize quality of life for patients, caregivers, and families.
Responsibilities
- Provide skilled patient care and home health services as outlined in the written plan of care.
- Assist a primary nurse in providing direct care.
- Initiate communication with physicians, managers, IPG members, HHAs, and other healthcare providers.
- Instruct primary caregivers, patients, volunteers, and employed caregivers in care provision.
- Attend patient conferences and participate in discussions to facilitate optimal care.
Requirements
- Completion of a Licensed Vocational Nursing Education Program from an accredited school.
- Minimum of one year of LVN experience, preferably in an acute care setting.
- Experience in a home health/hospice setting preferred but not required.
- Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners.
- Valid California certification in Intravenous Therapy & Blood Withdrawal or must attend and pass class within 90 days of hire.
