About the Role
The Care Coordinator is part of the Amplar Preventative Health Team, supporting individuals with complex and chronic care needs in the community through various service delivery modes like video, telephonic, and digital programs. This role empowers participants to self-manage their health by utilizing holistic health knowledge, focusing on chronic health management, health literacy, and facilitating health behavior change through health coaching.
Responsibilities
- Support people with complex and chronic care needs in the community through video and telephonic delivery or digital programs.
- Utilize holistic knowledge and background in health, specifically chronic health management, to support health literacy.
- Empower participants to self-manage their health.
- Facilitate health behavior change and care coordination in the delivery of health coaching.
- Deliver seamless and effective support services to people with complex and chronic care needs.
- Apply strong clinical knowledge and translate care plans into practice.
- Utilize a strong understanding of chronic disease management practices, including workflows across the care team.
Requirements
- Registered Nurse or Allied Health Professional (Occupational Therapist, Physiotherapist, Dietician, Exercise Physiologist, Podiatrist and Psychologist) with relevant registration and experience with chronic disease management.
- Experience in primary health care and care planning.
- Must have a suitable office area from which to work and a reliable internet connection.
- Availability to work Friday is highly desirable.
- Previous experience delivering care virtually and digitally is desirable but not essential.
- Understanding of health behavior change methodology and its application in practice is desirable but not essential.
