About the Role
Extended Home Care is seeking an experienced Speech Language Pathologist to provide home visits for individuals with intellectual and developmental disabilities (IDD) in The Bronx, servicing all five boroughs. The role involves assessing patients, developing and implementing treatment plans, and collaborating with healthcare teams to improve patient function and independence.
Responsibilities
- Assess patient’s speech and language abilities and deficits.
- Prepare initial assessments and perform periodic reassessments.
- Develop a plan of care in accordance with the authorized practitioner’s plan of treatment and agency policies.
- Provide rehabilitative services for speech and language disorders to meet established treatment goals.
- Select and use appropriate diagnostic and therapeutic techniques and materials.
- Document treatment type and patient’s response on clinical progress notes.
- Complete case conferences, prepare discharge summaries, and complete documentation on appropriate forms.
- Participate in the review, revision, and implementation of patient care policies and procedures related to speech-language pathology.
- Instruct other healthcare personnel, patients, and caretakers in methods to assist patients in home therapy programs.
- Participate in the development and periodic revision of patient care forms.
- Collaborate with other health care team members for continuous coordination.
- Participate in inter-disciplinary and case conferences, staff meetings, and in-service programs.
- Participate in the agency’s Quality Assurance Performance Improvement/PIP as assigned.
- Demonstrate sound judgment by taking appropriate actions regarding suspected violations of corporate compliance regulations.
- Report all suspected violations to supervisor, Compliance Officer, or Compliance Hotline.
- Perform other duties as assigned.
Requirements
- Master’s degree and Certification of Clinical Competence in speech pathology granted by American Speech Language and Hearing Association.
- License and current registration to practice as a Speech-Language Pathologist in New York State.
- One full year of supervised institutional experience is preferred.
- One year of home care experience is preferred.
- Meets state health status requirements.
- Available after 3pm.
