About the Role

Responsible for the day-to-day operations of Ryan Health Network Sites HIV Services programs. Provides direct supervision to Case Managers, Care Technicians, Data Entry staff, and Peer Educators. Ensures patients with targeted chronic conditions receive the highest quality care and support services, focusing on those over 50 experiencing health disparities. The role ensures the Over 50 program meets quality indicators and compliance, and connects patients at risk of falling out of care to continuous services. This work supports the mission and goals of Ryan Health.

Responsibilities

  • Supervise and coordinate case management, case-finding, monitoring, and follow-up activities.
  • Recruit, hire, onboard, train, schedule, monitor attendance, evaluate performance, and provide disciplinary action and day-to-day support to staff.
  • Facilitate weekly case conferences and consult with interdisciplinary team members and external service providers to develop unified service plans.
  • Analyze program data, prepare program reports, conduct chart reviews, monitor program outcomes, and provide recommendations for quality improvement.
  • Assist with program planning, development, and implementation.
  • Develop and maintain partnerships with external agencies.
  • Collaborate with fiscal leadership to support budget management and monitor program projections.
  • Coordinate staff and program operations, including case assignments, admissions, discharges, and ongoing case monitoring.
  • Participate in meetings with primary care and interdisciplinary staff.
  • Identify staff training and professional development needs, and provide ongoing coaching, supervision, and support.
  • Participate in professional development opportunities and remain current on evidence-based practices.
  • Serve as a super-user for all electronic systems utilized by program staff.
  • Coordinate staff trainings and develop internal training materials and procedures.
  • Participate in patient eligibility assessments, review staff assessments, and provide clinical recommendations.
  • Assist with proposal development, grant writing, abstracts, presentations, and new program initiatives.
  • Support the development and implementation of network-wide in-service training programs.
  • Assist staff and administrative monitors in functioning effectively as members of the interdisciplinary care team.
  • Attend and actively participate in required meetings, case conferences, trainings, and supervision sessions.
  • Maintain accurate and timely documentation related to supervisory activities, meeting minutes, quality improvement initiatives, and clinical operations.

Requirements

  • Three years of experience working with chronic care populations in a medical setting, including behavioral health and HIV populations.
  • Three years of clinical and administrative experience in social work or public health, preferably within a medical setting.
  • Working knowledge of Microsoft Office Suite, as well as database management.
  • Probationary period of 90-days of actual workdays.
  • Work conditions inherent in ambulatory health care and outreach settings.
  • Some traveling to various community sites required.
  • Flexible work schedule may include evenings or weekends.
  • Must be able to perform the essential functions of the job, with reasonable accommodations.
  • Adequate preparation and precaution is necessary due to potential exposure to infectious diseases.
