About the Role

The Nurse Residency Program (RN) is a structured transition-to-practice program designed to support newly licensed Registered Nurses in developing clinical competence, confidence, and professional judgment within the PACE care model. Through hands-on experience, mentorship, and interdisciplinary collaboration, Nurse Residents will gain exposure to home-based care, geriatric medicine, and coordinated care delivery. Upon successful completion, participants may transition into other clinical nursing roles.

Responsibilities

  • Deliver nursing care to participants in accordance with individualized care plans, physician orders, and evidence-based nursing practice standards.
  • Conduct supervised home visits for wound care management, medication administration, participant education, and caregiver support.
  • Observe, assess, and document participant health conditions, reporting changes to the interdisciplinary care team and supervising nurse.
  • Support disease management and health promotion for participants with chronic conditions.
  • Participate in clinical training modules focused on geriatric care, chronic disease management, medication safety, infection control, and community-based care delivery.
  • Receive ongoing mentorship and clinical guidance from experienced nurses and clinical leadership.
  • Participate in case reviews, clinical discussions, and interdisciplinary team meetings.
  • Develop competency in clinical documentation and use of electronic health record systems.
  • Work closely with physicians, nurse practitioners, social workers, rehabilitation therapists, and other interdisciplinary team members.
  • Support care coordination efforts to ensure participants receive comprehensive, high-quality care.
  • Participate in care planning discussions and team-based decision making.
  • Maintain accurate and timely documentation in the organization's electronic health record system.
  • Adhere to all clinical protocols, regulatory requirements, and infection control standards.
  • Meet the competency standards established by the PACE organization prior to independent practice.

Requirements

  • Ability to travel to participant homes and clinical sites within the designated service areas.
  • Proficiency with computers and electronic health record systems.
  • Must be medically cleared for communicable diseases and have all required immunizations prior to direct participant contact.
  • Must meet all regulatory requirements applicable to the PACE program.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively within an interdisciplinary team.
  • Strong organizational and time management abilities.
  • Demonstrated compassion and commitment to patient-centered care.
  • Willingness to learn and develop clinical expertise.
  • Individuals must be able to sustain certain physical requirements essential to the job, including standing, sitting, lifting up to 50 pounds, bending, squatting, maneuvering stairs, walking, climbing, agility, fine motor skills, adequate sight, hearing, and motor skills for communication, and cognitive ability for decision-making.

  • Service Time 0-3 Months: $100,000.00
  • 4-6 months: $111,000.00
  • Post-Completion: $118,560.00 - $124,800.00
