About the Role

As an SLP in our Integrated Care Program, you’ll play a vital role in post-acute recovery, working alongside a collaborative team to deliver personalized, impactful care in the community. Support communication and swallowing where patients feel most comfortable — at home.

Responsibilities

  • Assessment and implementation of SLP interventions (i.e. swallowing assessments, dysphagia management, communication, etc.) in a home-based setting
  • Use of a collaborative and interdisciplinary team-based approach to care and services
  • Support a holistic and restorative approach to care (i.e. enhanced stroke pathway; cognitive communication supports, etc.)
  • Leverage our digital health platforms to support care delivery, collaboration, documentation, and intervention

Requirements

  • Registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
  • Valid driver's license and vehicle insurance
  • Focus on embracing person and family-centered care
  • Comfortable delivering care both in person and using virtual methods/technology
  • Passionate, empathetic, use good judgement and apply critical thinking when making client decisions
  • Excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
  • Flexible and have demonstrated skills in critical thinking that you can apply to a variety of situations
  • Value the role a therapist plays in keeping clients at home, where they want to be
