About the Role

Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in Austin, TX, and surrounding areas. PRN positions are available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans practiced in patients' homes and communities, using creative, evidence-based approaches to achieve durable outcomes.

Responsibilities

Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification

Requirements

Excellent planning, organization, writing, and time management skills are required
Ability to communicate with families, professionals, and co-workers is a must
