About the Role

Collage Home and Community Rehabilitation Partners is seeking experienced Therapeutic Recreation Specialists to provide patient-centered care in Austin, TX and surrounding areas. This role involves working with interdisciplinary teams to create integrated treatment plans practiced in patients' homes and communities, focusing on neurological rehabilitation for conditions like traumatic brain injuries, spinal cord injuries, and CVAs.

Responsibilities

Evaluation, treatment, and education of neuro-rehab patients including planning, organizing, and coordination of recreation activities, functional leisure skills, and leisure education
Assess capabilities, interests, and barriers
Monitor progress and recommend changes or modifications to the program as required
Perform home assessments and assist patients with the transition to home and community

Requirements

Excellent planning, organization, writing, and time management skills are required
Ability to communicate with families, professionals, and co-workers is a must
