About the Role
The Licensed Vocational Nurse (LVN) at Care Indeed delivers skilled, patient-centered nursing care across home and community-based settings. LVNs apply clinical judgment to support curative, rehabilitative, palliative, and preventative care goals, making real-time clinical decisions that influence patient outcomes.
Responsibilities
- Deliver skilled nursing care in home and community settings in partnership with the RN and interdisciplinary care team
- Use clinical judgment to carry out the plan of care and support curative, rehabilitative, palliative, and preventative treatment goals
- Observe, assess, document, and report patient status, including physical condition, mental status, symptoms, responses to treatment, safety concerns, and changes in condition using clear clinical terminology
- Support patients in self-care, mobility, body mechanics, and rehabilitation activities aligned with the medical care plan
- Recognize social, emotional, and environmental factors that influence patient well-being and provide care that promotes comfort, dignity, and safety
- Participate in care planning, team conferences, and ongoing learning that strengthen clinical skills and teamwork
- Complete accurate clinical and progress notes weekly, and as needed, using EMR systems such as WellSky through mobile or tablet platforms
- Provide clinical guidance and task delegation to Home Health Aides (HHAs) and Home Care Aides (HCAs) in alignment with the care plan and under RN oversight
Requirements
- Active California LVN license and current CPR certification
- Ability to work independently in client homes and facilities and make real-time clinical decisions within the scope of practice
- Proficiency in spoken and written English for documentation and care coordination
- Commitment to communicating with clients in the language that supports their comfort and understanding whenever possible
- Experience with electronic medical records, including systems such as WellSky, using mobile or tablet devices for timely, compliant documentation
- Reliable transportation and the ability to travel between client locations
- Physical ability to lift up to 50 lbs and perform essential nursing duties in varied environments
- Successful completion of Live Scan fingerprint background clearance is required prior to employment
- Candidates may be hired before obtaining CPR certification, but must secure certification within two weeks of hire and maintain it throughout employment
