About the Role
As a member of our Transitional Residential Treatment Facility team, you'll help clients build life skills, manage their challenges and grow their personal strengths with the ultimate goal of successfully transitioning to greater wellness and independence.
Responsibilities
- Ensure participant’s Electronic record and paper file is complete and up-to-date
- Ensure goals are being worked toward and program expectations are being met
- Provide participant updates at Team Meetings
- Create Team Approaches and/or Contract of Residency
- Follow through with primary responsibilities when a client’s Primary Worker is unavailable
- Build on client’s strengths, abilities and resources
- Assess social and environmental factors impacting the client
- Assist clients to develop and coordinate an Individual Rehabilitation Plan (IRP) based on their strengths, abilities, needs and preferences
- Review and revise IRP with client every four-six weeks, or more frequently if deemed necessary
- Motivate and support the client to work towards achievement of their IRP goals
- Include and encourage clients’ natural supports (e.g. family, significant friends) in the IRP, clarifying the roles each plays towards clients’ rehabilitation
- Regularly solicit feedback from the client and relevant community supports regarding participant’s mental health or addiction and progress towards his/her goals
- Completion of frequent Mental Health Status Exams within the first week of the client’s admission, and subsequently at a minimum interval of once per month
- Create or update client Crisis Plans
- Medication monitoring and education
- Support clients with medication routine
- Completion of Medication Reconciliation procedure
- Ongoing liaison with Nursing Staff Team to ensure a firm understanding of the clinical aspects of the program
- Advocate for and support clients in accessing community resources
- Champion each client’s capacity for independent community living
Requirements
- College diploma or university degree in the social science field
- G driver’s licence
- Access to a vehicle for personal transportation
- Current, satisfactory Police Vulnerable Sector Check (PVSC)
- Demonstrated ability to attend work regularly
- Work experience in the addiction and/or mental health field
- Experience working in a residential supportive housing environment
- Knowledge of safe food handling procedures
- Knowledge of Psychosocial Rehabilitation
- Crisis intervention and prevention skills and experience

Salary/Compensations
- Starting rate $27.14 per hour (Level 1 – Direct Service)
- $30.01 per hour for Level 1 – Direct Service, Regulated
- Up to $34.41 per hour for non-regulated (Level 8)
- Up to $37.26 per hour for regulated (Level 8)
- Premiums of $2.00 per hour for eligible Evening/Night (8:00pm – 7:00am) and Weekend Shifts
