About the Role

As a member of the Transitional Residential Treatment Facility team, you will support clients in building life skills, managing challenges, and growing personal strengths to achieve greater wellness and independence.

Responsibilities

  • Act as a Designated Primary Worker for an assigned caseload
  • Maintain accurate and up-to-date electronic and paper client records
  • Monitor progress toward client goals and program expectations
  • Provide participant updates during team meetings
  • Create team approaches and residency contracts
  • Assess social and environmental factors impacting clients
  • Develop and coordinate Individual Rehabilitation Plans (IRP) based on client strengths and needs
  • Review and revise IRPs every four to six weeks
  • Motivate and support clients in achieving IRP goals
  • Engage clients' natural supports in the rehabilitation process
  • Solicit regular feedback from clients and community supports regarding progress
  • Complete frequent Mental Health Status Exams
  • Create and update client crisis plans
  • Perform medication monitoring, education, and reconciliation
  • Liaise with the nursing staff team regarding clinical aspects of the program
  • Advocate for and support clients in accessing community resources
  • Champion client capacity for independent community living

Requirements

  • College diploma or university degree in a social science field
  • Valid G driver’s licence
  • Access to a vehicle for personal transportation
  • Current, satisfactory Police Vulnerable Sector Check completed within the last six months
  • Demonstrated ability to attend work regularly

Salary/Compensations

  • $27.14 per hour starting rate (non-regulated)
  • $30.01 per hour starting rate (regulated)
  • $34.41 per hour maximum (non-regulated)
  • $37.26 per hour maximum (regulated)
  • $2.00 per hour premium for eligible evening, night, and weekend shifts
