About the Role

The Community Liaison will build and strengthen relationships with community partners to expand outreach and engagement. The ideal candidate will design and implement strategic initiatives to grow and sustain community presence. This is a critical role focused on identifying, operationalizing, and measuring the success of community partnerships related to social determinants of health, working closely with the community and Vynca’s Enhanced Care Management team. The role requires a self-motivated, energetic, compassionate individual with a strong background in community support and patient engagement, driven to help the underserved.

Responsibilities

  • Build and maintain relationships with community stakeholders, including resource centers, medical facilities, and advocacy groups.
  • Drive referrals into Vynca care programs, primarily Enhanced Care Management.
  • Continuously expands knowledge and understanding of community services and resources; maintains frequent contact with community agencies serving vulnerable populations.
  • Understand local market dynamics and agency strengths.
  • Develop and execute strategic marketing plans.
  • Identify and communicate cultural issues affecting members.
  • Track sales activities and conduct presentations.
  • Support community events and maintain high customer service standards.
  • Serve as a liaison between patients and practices.
  • Strengthen referral partnerships through outreach efforts.

Requirements

  • 2 years of experience in healthcare sales or at a community-based organization required.
  • Experience working with underserved communities and in healthcare.
  • Experience & comfort using technologies such as a computer, telephone, and various types of electronic health record (EHR) platforms to document patient interactions and schedule patients for appointments.
  • Experience working in a CRM, Salesforce experience preferred.
  • Must possess strong organizational skills.
  • Ability to work independently.
  • Proficient with Microsoft and Google applications.
  • Strong communication, speaking, and presentation skills.
  • Strong motivational skills, including conflict/dispute resolution techniques.
  • Must have access to a reliable internet connection.
  • Valid driver's license and reliable, insured, and registered automobile.
  • A background check, which may include a drug test or other health screenings depending on the role, will be required prior to employment.
  • Compliance with federal law requires identity and work eligibility verification using E-Verify upon hire.
  • Employees in patient, client, or customer-facing roles must be vaccinated against influenza. Requests for religious or medical accommodations will be considered but may not always be approved.
