About the Role

The Intensive Case Manager will demonstrate a compassionate and caring nature, working with clients experiencing mental illness and substance use to develop trust and support their recovery process. This role is grounded in regular assessment of client-centered needs and collaborative goal setting, embodying the mission, vision, and values of Good Shepherd.

Responsibilities

• Provide direct service to clients experiencing mental illness and substance use.
• Assess client-centered needs and collaborate on client-driven goals.
• Demonstrate a positive work ethic and role model the mission, vision, and values.
• Provide service in a dignified, culturally competent, and respectful manner.
• Maintain a caseload within provincial guidelines for Intensive Case Management.
• Engage in the intake process, including liaison with Access Point, interviewing clients, and utilizing assessment tools.
• Complete client summary reports, discharge/transfer reports, and maintain client files and progress notes.
• Provide resource information, community facilitation, and advocacy for clients.
• Complete referrals as required.
• Support clients in achieving goals that enhance their quality of life using a psychosocial rehabilitation approach.
• Liaise with clients' formal and informal supports with client consent.
• Assist clients in enhancing and developing necessary life skills.
• Help clients maximize their competencies to become more self-sufficient.
• Work from a strengths-based perspective within an empowerment framework.
• Keep the Program Manager informed of client developments and changes.
• Observe all health and safety rules and work in compliance with the Occupational Health and Safety Act.

Requirements

• University Degree in Social Work, Nursing, Occupational Therapy or related field.
• Addictions Certificate from a recognized college.
• Two years' work-related experience, particularly with persons experiencing problematic substance use and mental health challenges.
• Extensive knowledge of the social service system and community resources.
• Expertise providing counselling and case management to individuals with a complex history of substance use and mental health concerns.
• Crisis intervention and risk assessment skills.
• Well-developed problem-solving abilities and interpersonal skills.
• Demonstrated knowledge of crisis intervention de-escalation strategies and interventions.
• Ability to remain calm in crisis situations.
• Demonstrated ability to recognize and respond to the emotional, psychological and physical needs of individuals reacting to a crisis situation.
• CPR certification or willingness to be certified.
• Committed to anti-racist/anti-oppressive practice and to the mission and values of Good Shepherd.
• Demonstrated understanding of workplace health and safety practices and employee responsibilities under current legislation.
