About the Role
The Hospice Clinical Manager is responsible for ensuring that hospice patient care is coordinated and managed appropriately, overseeing the delivery of care and services, and managing clinical personnel.
Responsibilities
- Receives case referrals and reviews patient information to determine hospice needs.
- Assigns appropriate hospice personnel to cases.
- Conferences with the medical director regarding patient eligibility for services.
- Reviews and evaluates cases through home visits, conferences, and record reviews.
- Guides clinicians to ensure effective performance and quality hospice services.
- Assists clinicians in establishing therapeutic goals, setting priorities, and developing care plans.
- Attends case conference meetings to coordinate care and discuss interdisciplinary group involvement.
- Assists the Chief Clinical Officer in planning, implementing, and evaluating in-service and continuing education programs.
- Assists in formulating, revising, implementing, and evaluating organizational policies, procedures, goals, and objectives.
Requirements
- Current NH RN license.
- Previous experience in a hospice/home care setting.
- Two (2) years of management or supervisory experience.
- Proven ability to work within an interdisciplinary setting.
- Complies with accepted professional standards and practice.
- Excellent observation, good nursing judgment, and communication skills.
- Understands hospice philosophy and issues of death/dying.
- Proof of flu vaccination.
- Reliable transportation.
- Valid driver's license.
- Automobile insurance.
- Physical exam within the last year.
- Two-step TB test.
