About the Role
The Case Coordinator provides clients with comprehensive assessments, develops care plans to meet client goals, and supports clients and caregivers in managing care budgets and resources. This role supports client self-management within VON policies and frameworks, focusing on individuals with developmental disabilities receiving residential support.
Responsibilities
- Interpret and complete standardized assessment tools (e.g., interRAI CHA, Supports Intensity Scale, Application for Developmental Services).
- Complete client assessments using conversation-based methodology.
- Develop and conduct comprehensive therapeutic rapport.
- Identify and prioritize environmental risks to client health and safety.
- Develop comprehensive care plans, interpreting clinical assessment protocols.
- Facilitate access to client-identified services and navigate appropriate resources.
- Make referrals to community and health resources as needed.
- Consult with partners in care, with client consent, to ensure shared understanding and appropriate interventions.
- Monitor client progress and adjust care plans accordingly.
- Review client care plan expenditures and support clients in managing budgets, advocating for additional resources.
- Access and review assessments.
- Collect and report relevant data to the Manager on program outcomes.
- Adhere to privacy policies and standards for Community Support Service or Developmental Services agencies.
- Positively and professionally represent providers, stakeholders, and agencies to clients.
- Mediate and problem-solve with multiple parties in various contexts.
- Coordinate and provide Family Home support for a caseload.
- Ensure referrals and support are provided in emergency and urgent situations.
- Participate in the screening and matching of new Family Home Providers.
- Promote the goals and values of VON and their role as an integrated community care provider.
- Promote a safe and healthy workplace in accordance with the Occupational Health and Safety Act.
- Abide by all VON policies, work practices, and confidentiality policies.
- Work in collaboration with other staff in a team approach to service delivery.
- Engage frequently with Administrative Support, Community Support and Home Care program Coordinators and Manager.
- Network and collaborate with regional partners, Health Care and non-traditional providers and agencies.
- Be available for weekend assessment coverage.
- Work flexible hours, including evenings and weekends, to meet client and family needs.
- Respond to emergency client situations 24/7 (on call).
Requirements
- University Degree or College Diploma in a Health or Social Services field.
- Minimum of 3 years related experience in a home and community care organization or social services program.
- Experience in case management.
- Experience in building external and community relationships.
- Demonstrated experience in completing validated common assessment tools and maintaining competency.
- Broad working knowledge of community resources within the defined catchment area.
- Knowledge and skills in comprehensive assessment and the ability to maintain proficiency.
- Working with volunteers and seniors is an asset.
- Strong attention to detail and accuracy.
- Skilled in communication (verbal, written, listening), rapport building and relationship management.
- Computer proficiency in Windows OS and Microsoft Office programs.
- Ability to use analytical skills and understand/interpret client assessment data.
- Ability to effectively manage multiple tasks, prioritize, meet deadlines, and work well under pressure.
- Knowledge of social and developmental service sectors may be required.
- Demonstrated skills in leadership and team building.
- Ability to educate clients and families on available program resources.
- Strong time management and organizational skills.

- Must have access to a personal vehicle for business use.
- Possess a current driver’s license.
- Possess proof of vehicle insurance.
- A current and original copy of a satisfactory Criminal Records Check.
- A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
- The use of Personal Protective Equipment (PPE) may be required.
- Requires a detail-oriented approach in a dynamic environment.
- Requires physical activity including lifting, carrying, bending, reaching, kneeling, walking, sitting, standing, and climbing stairs.
