About the Role

The Aging at Home (AAH) program provides services to seniors aged 55+ to help them live independently. The AAH Coordinator will support clients with services like friendly visiting, home support, and accessing transportation for medical appointments. This role also involves coordinating contracted services for lawn care and snow removal.

Responsibilities

  • Ensure the Aging at Home program is delivered in the spirit and philosophy of the Métis Nation of Ontario’s Statement of Prime Purpose.
  • Provide client services including home service or friendly visits, service support assessments, and service coordination with outside agencies.
  • Coordinate and track contracted services for lawn care and/or snow removal.
  • Support clients in accessing gas cards or third-party transportation for specialized medical appointments.
  • Assist clients in accessing services through referrals.
  • Record client services in the Online Health and Access Tracking System (OHATS) and Excel tracking documents.
  • Maintain confidentiality and adhere to privacy legislation.
  • Perform financial reporting specific to the program site and budget.
  • Attend training and meetings outside of Métis Nation of Ontario working hours.
  • Perform administrative duties and other tasks as instructed by the Program Supervisor.
  • Participate in ongoing professional development activities as time and financial resources permit.
  • Fulfill all other responsibilities as determined by management of the Healing and Wellness Branch.

Requirements

  • Experience working with volunteers in a non-profit environment.
  • 1 or more years of experience in client services.
  • 1 or more years of experience working with the elderly population (55+).
  • Proficiency with computer programs including Outlook, Excel, Word, and PowerPoint.

Experience Level

  • 1 or more years’ experience in client services experience
  • 1 or more years’ experience working with elderly population (55+)

Education Level

  • Degree or diploma in social services
