This role reimagines therapy, integrating it into EmpowerMe 360, a seamless model connecting therapy, wellness, and medical providers for residents. As a trusted guide for seniors, you will help them live stronger, safer, and more independent lives. Your impact directly contributes to residents' independence by helping them move with confidence, reduce fall risks, and master daily tasks.
Responsibilities
- Provide physical, occupational, or speech therapy and wellness services for older adults
- Conduct assessments, create personalized care plans, and deliver evidence-based interventions collaboratively
- Support care coordination through wellness check-ins, caregiver communication, and telehealth support
- Maintain professional and ethical standards aligned with company initiatives
- Contribute to program development and continuous quality improvement
- Help residents maximize independence, confidence, and satisfaction
Requirements
- Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP)
