About the Role

The Mary Lee Foundation, a nonprofit, seeks a Case Manager to enhance patient management quality, maximize satisfaction, and promote cost-effectiveness. This role involves managing the full patient care cycle, including assessing, planning, implementing, monitoring, and evaluating client health and human services needs.

Responsibilities

  • Coordinate and provide safe, timely, effective, efficient, equitable, and client-centered care.
  • Manage case assignments, draft service plans, review progress, and determine case closure.
  • Support clients in achieving wellness and autonomy.
  • Facilitate various care aspects, including coordination and information sharing.
  • Advocate for patients to help them make informed decisions about clinical status and treatment.
  • Develop effective working relationships and cooperate with the medical team throughout case management.
  • Arrange abeyances with social services, health, and governmental agencies.
  • Interact with patients to monitor progress and ensure satisfaction.
  • Record case information, complete necessary forms accurately, and produce statistical reports.
  • Promote quality and cost-effective interventions and outcomes.
  • Assess and address motivational and psycho-social issues.
  • Adhere to professional standards, protocols, rules, and regulations.
  • Supervise direct care staff.
  • Participate in an on-call rotation one full week every four weeks.

Requirements

  • 2 years of experience working with individuals with disabilities.
  • Proven working experience in case management (e.g., nurse, medical, mental health care manager).
  • Valid Texas Driver License or ability to acquire within 30 days of employment.
  • No bars to employment per Health & Safety Code §250.006.
  • No bars to employment per Texas Administrative Code, Title 40, Part 1, Chapter 3, §3.201.
  • Name not on Texas or Federal Office of Inspector General’s Exclusions List.
  • Ability to perform physical duties: lifting/carrying at least 50 lbs, climbing stairs, bending, stooping, driving.
  • Ability to perform physical and mental requirements: seeing, hearing, speaking, writing clearly.
  • Ability to perform occasional reaching, stooping, kneeling, crouching, crawling.
  • Ability to perform frequent sitting, standing, walking for long periods, climbing stairs, walking on inclines and uneven terrain.
  • Ability to perform frequent lifting, bending down, and/or moving up to 50 pounds.

Skills

  • Excellent knowledge of case management principles, healthcare management, and reimbursement.
  • Experience with psychological aspects of care.
  • Effective communication skills.
  • Excellent organizational and time management skills.
  • Familiarity with professional and technical emerging knowledge.
  • Problem-solving skills and ability to multi-task.
  • Compassion and teamwork skills.

Experience Level

  • 2 years of experience working with people with disabilities.
  • Proven experience in case management.
  • Supervisory experience preferred.

Education Level

  • Certified Case Manager (preferred).
