About the Role

Provides personal care services to On Lok participants receiving home care and/or day center services, functioning as a Personal Care Attendant as per PACE regulation (42 CFR Part 460).

Responsibilities

  • Perform duties of a Caregiver 1
  • Provide onboarding and ongoing training for other Caregiver-1s and 2s using manuals and formal training materials
  • Actively participate in departmental meetings and family meetings as needed
  • Perform other related duties as assigned

Requirements

  • Minimum one-year experience working with frail or elderly
  • Minimum one-year experience working in home or clinic setting preferred
  • Flexibility and positive attitude working across multiple settings
  • Ability to work independently and as part of an interdisciplinary team
  • Knowledge, use, and explanation of safety equipment for lifting
  • Ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and repeat motions of the wrist and hands
  • Visual acuity
  • Ability to lift/move weight within individual physical ability and use appropriate lifting devices or request assistance
  • Basic English reading and writing
  • Bilingual ability may be required in specific assignments and daily participant interaction

Skills

  • Understanding of PACE model
  • Basic computer skills
  • Ability to use or learn multiple applications such as email and electronic health record

Work Type

  • Regular position (20+ hours/week)

Experience Level

  • Minimum one-year experience working with frail or elderly
  • Minimum one-year experience working in home or clinic setting preferred

Education Level

  • HHA or CNA required
  • Current Basic First Aid and CPR certification

Salary/Compensations

  • Sign-On Bonus: $1,500
  • Pay range expressed as an hourly rate
