About the Role
This position provides direct case management to clients, evaluating needs and developing service plans for approximately 25-30 active clients.
Responsibilities
- Provide direct client services including individual counseling, case management, advocacy, and accompaniment.
- Ensure Entry Case Management Services at Division Circle Navigation Center comply with standards, regularly monitoring client activity related to achieving personal and programmatic goals.
- Conduct new client screenings, reviewing applications and interviewing new clients.
- Enter client data into the ONE database within 48 hours.
- Create a service plan for each client with key issues prioritized.
- Partner with service providers to coordinate care and meet client service plan goals.
- Assist clients in maintaining stability by offering support and linkage to services.
- Problem-solve with clients to determine the best assistance methods.
- Observe clients passively to identify issues requiring intervention.
- Facilitate meetings and support groups as needed.
- Develop client exit surveys and case summaries to support outcomes and program analysis.
- Maintain accurate and timely electronic client records, individual case files, daily logs, and service statistics.
- Participate in regular staff meetings, including case management meetings and program development.
- Participate in ongoing staff development and training.
- Perform other appropriate duties as assigned.
Requirements

- Proficient in MS Office including Word, Outlook, and Excel
- Working knowledge of community resources
- Familiarity with SF homeless shelter systems and services
- Commitment to open, direct, and non-violent communication
- Ability to de-escalate crisis situations
- Ability to make decisions under pressure
- Average reporting skills including data entry

Experience Level
- Two years’ experience providing services to chronically homeless preferred
- Relevant comparable experience will be considered
Education Level
- BA degree in social services, psychology, or another related field
- Equivalent work experience may be accepted in lieu of a degree
Salary/Compensations
- $30.00 per hour
