About the Role
Jericho Road Community Health Center seeks a full-time Healthy Steps Program Specialist to work across its Barton, Broadway, and Doat locations.
Responsibilities
- Conduct team-based well-child visits with parents and children (ages 0-3) alongside medical professionals.
- Provide consultations to families regarding developmental and behavioral concerns for children (ages 0-3).
- Collaborate with the Healthy Steps team to implement program core components, including universal child development and SDOH screening.
- Ensure positive parenting and early learning guidance information is available to all practice staff.
- Consult medical professionals and staff on early childhood development, infant mental health, and trauma-informed practice.
- Provide referrals and track follow-up to connect families with community resources.
- Work with pediatric providers on care coordination, goal setting, counseling, coaching, and education about child development.
- Conduct home visits and accompany families to medical, specialty, and community agency appointments as needed.
- Facilitate Parents'/Caregivers' groups.
- Document all patient clinical activity and care coordination in electronic medical records.
Requirements
- Master’s degree in psychology, social work, counseling, early childhood education, or a related field, or equivalent experience.
- Licensed professional preferred.
- Experience and knowledge of early childhood growth and development, parent-child relationships, child health, infant mental health, and family systems.
- Experience evaluating the growth and development of infants and children under three years of age.
- Demonstrated commitment to working with underserved populations in a community setting with diverse cultures.
Skills

- Ability to collect and enter data for program management, evaluation, and reporting.
