About the Role

Jericho Road Community Health Center seeks a Hospital Medicine Physician to contribute to a global mission of influencing health and wellbeing. As a federally qualified health center (FQHC), the organization is dedicated to caring for communities, advocating for systemic health equity, and making fundamental changes, with opportunities to engage in global clinics.

Responsibilities

  • Provide high quality inpatient medical care for adult hospitalized patients.
  • Provide direct clinical supervision and teaching to the Family Medicine Resident team.
  • Perform comprehensive history and physical examinations, admissions, daily progress evaluations, and discharge planning with the family medicine resident team.
  • Manage acute and chronic medical conditions collaboratively.
  • Ensure timely, evidence-based, cost-effective inpatient care.
  • Coordinate care transitions with outpatient providers, specialists, case management, social work, nursing, rehabilitation, and ancillary services.
  • Maintain electronic medical records efficiently and accurately, including timely completion of documentation, orders, discharge summaries, and medication reconciliation.
  • Participate in interdisciplinary rounds and collaborate with nursing and hospital staff to optimize patient outcomes and experience.
  • Respond to changes in patient condition and participate in rapid evaluation and escalation of care as appropriate.
  • Participate in quality improvement, patient safety, and utilization management activities using a data-driven approach.
  • Attend required staff, provider, and hospital committee meetings, unless clinical responsibilities prevent attendance.
  • Provide patient care holistically, addressing physical, mental, emotional, social, and spiritual health.
  • Support safe discharge planning and reduce avoidable readmissions through patient education and coordination of follow-up care.
  • Maintain electronic medical records efficiently and maximize available resources.
  • Complete required hospital documentation, billing, coding, and compliance activities accurately and timely.
  • Participate in quality improvement, patient safety, and regulatory readiness activities.
  • Maintain current certifications, including all mandatory annual training, ACLS/BLS, and credentialing requirements.
  • Adhere to professional code of ethics and comply with hospital, state, federal, CMS, Joint Commission, and organizational requirements.
  • Collaborate with interdisciplinary clinical and program teams across hospital and community settings.
  • Foster respectful and collaborative relationships with physicians, advanced practice providers, nurses, consultants, support staff, patients, and families.

Requirements

  • Physician licensed to practice in New York State prior to employment start date.
  • Board Certified as a Physician in Family Medicine or Internal Medicine.
  • Experience in hospital medicine, inpatient medicine, emergency medicine, critical care, or related field preferred.
  • Current BLS certification required.
  • ACLS certification preferred or required within designated timeframe.
  • Experience working with low-income, diverse, multilingual populations highly desired.
