About the Role

As a trusted guide and advocate, this role serves as the central support for youth and families seeking care coordination and community connections. It involves close collaboration with an extended ECM team across counties, partnering with managed care plans, and building strong relationships to ensure youth and families receive necessary services to thrive.

Responsibilities

• Engage in outreach to enroll new clients in ECM and build rapport with enrolled members.
• Complete comprehensive assessments and develop care plans for each enrolled member.
• Organize care activities and maintain regular contact with outside providers to ensure coordination, including county substance use disorder and specialty mental health providers.
• Manage referrals, coordination, and follow-up for needed services and supports.
• Provide education and identify support needs for youth and their family or other caregivers.
• Deliver services in person and/or via telehealth to meet each enrolled member’s care plan needs.
• Utilize crisis communication and de-escalation techniques as per Seneca's crisis response training, potentially including physical management of clients.
• Engage in community outreach, identify supports and resources, and build relationships with community providers.

Requirements

• Bachelor's degree OR
• Associate degree or equivalent + 1 year of applicable work experience OR
• High school diploma/GED + 2 years of applicable work experience
• Applicable experience includes paid or unpaid work with youth, families, or in the social services field.
• At least 21 years of age.
• Ability to drive within the community to meet with clients, attend meetings, or perform job duties at multiple locations within a specific region.
• TB test clearance.
• Fingerprinting clearance.
• Any other state/federal licensing or certification requirements.
• Valid driver's license.
• Clean driving record.
• Insurability through Seneca Family of Agencies’ insurance policy.
• Own vehicle.
• Current automobile insurance.
• Prior case management and community-based experience preferred.
• Lived or family experience navigating medical and mental health, probation, child welfare, and/or education systems and resources preferred.

• Community-based driving within a specific region
• Travel up to 90 miles
