About the Role
IOA leads revolutionary healthcare models, transforming lives and enhancing communities by helping aging adults and adults with disabilities maintain quality of life. With over 23 programs, IOA aids seniors in maintaining health, well-being, independence, and community participation. This role involves assessing clients with multiple medical and psychosocial needs, then planning and monitoring services to ensure quality care.
Responsibilities
- Conduct comprehensive assessments and ongoing re-assessments of client psychosocial, physical, mental health, environmental, and spiritual needs.
- Write comprehensive assessments.
- Develop and initiate client-centered, comprehensive Community Living Plans consistent with program guidelines and policies.
- Conduct home visits, acute hospital and skilled nursing facility visits, and escort clients to medical appointments as clinically indicated.
- Identify, arrange, and monitor appropriate community services with knowledge of Medicare, Medi-Cal, and other entitlement programs.
- Establish and maintain a care management relationship with clients and their informal support network, offering respect, dignity, and support.
- Provide crisis intervention, advocacy, problem solving, and therapeutic interventions.
- Meet with clients at least monthly, or more often as clinically indicated.
- Review and modify Community Living Plans on an ongoing basis.
- Document all case management activity via progress notes within 24-48 hours, adding new problems to the Community Living Plan as needed.
- Maintain required paperwork and follow a clear, concise, and consistent charting system for continuity of care.
- Conduct ongoing evaluation for client Purchase of Service needs and follow-up to ensure timely service provision.
- Educate clients and informal support networks about resources.
- Establish and maintain open and effective communication with community providers, including physicians and other healthcare and social service workers.
- Provide appropriate information on all significant aspects of individual client care and program operations, maintaining confidentiality.
- Monitor the quantity and quality of services provided by other involved providers.
- Continuously evaluate clients’ ability to remain safely at home and coordinate placement as appropriate, according to program guidelines.
- Collaborate with the client, caregiver, and involved services to terminate clients when appropriate, documenting the process as required.
- Participate in research studies and data collection as required.
- Participate in and promote ongoing efforts towards Continuous Quality Improvement.
- Attend and actively participate in team and program meetings, activities, and problem-solving endeavors, contributing to open communication.
- Utilize supervision appropriately, maintaining open communication and providing updates on caseload activity.
- Actively incorporate the ethical and legal standards of the National Association of Social Workers into all interactions.
- Understand and apply the regulatory and procedural requirements of the Institute on Aging.
- Attend continuing education classes and/or in-service training to increase knowledge, skills, and attitudes related to case management, gerontology, family and community systems, and the Community Living Fund client population.
- Perform all other reasonably related responsibilities as assigned.
Requirements
- One year working with disabled adults and/or older adults.
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
- Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders is highly desired.
- Exceptional communication and presentation skills relating to functionally impaired adults and older adults, their support systems, and teams of health professionals.
- Demonstrated case management skills and experience in the community health care delivery system.
- Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.

Experience Level
- One year working with disabled adults and/or older adults
- Minimum of two (2) years of relevant social work experience (if qualifying with BA or BS)
Education Level
- M.S.W. (Masters in Social Work)
- Masters level degree such as an MPH or MFT with additional or specialized work experience (e.g., psychology, counseling, geriatrics)
- BA or BS in Social Work or another appropriate major (with minimum two years relevant social work experience)
